Instructions for Submission of Application Form and Required Documents:

1. 1. Download and complete the application form.

   2. Email the completed application form together with the required documents in electronic format.

   3. Arrange all documents in the specified order and compile them into a single PDF file.

   4. Use the following file name and email subject format:
      Lastname, Firstname - Documentary Requirements
      (Example: DELACRUZ, JUAN - DOCUMENTARY REQUIREMENTS)

   5. Send the compiled PDF to the appropriate email addresses based on your location:

2. Learner Permanent Record (formerly known as Form 137) and SHS Form 138
3. Essay( minimum 300 words; font style-Times New Roman, Font Size- 12 )
   • Address in your essay: "Why is having a college education important to me"
4. 2pcs 1x1 IDphoto(white background) and 2pcs 2x2 IDphoto (white background)
5. (1) Letter of recommendation from teacher ( preferably typed on letterhead of school)
6. Tuition Fee Assessment (university applied)
7. Official Entrance Exam Result ( from university applied)
8. Birth Certificate
9. Proof of Income(any of the following)
   • Recent Income Tax Return
   • Affidavit of Tax Exemption by BIR
   • Indigency (issued by MSWDO, DSWD, CSDW; not by barangay)
10. Proof of recent bills( Electricity and Water )
